The water bag with a puppy inside dangles out !
Healthy puppies are a joy to deliver. When the water bag appears, it
is always an emergency to rush to your veterinarian to get a
Caesarian section before it ruptures and preferably before two hours
has lapsed.
Many times, the breeder or owner takes a long time to reach the
veterinarian. More than half an hour and in one case, one and
a half hour since notifying me that he or she needs a Caesarian
section!
 The
Shih Tzu arrived after an hour. The water bag was intact. As I put
her under the gas anaesthesia, a whitish puppy head appeared in the
water bag!
It disappeared again when the bitch stopped contraction. She
was in the 3rd (final) stage of labour. Should I break the
water bag and pull out the puppy?
It was not a simple yes or no! If the puppy was large and had stuck
in the birth canal, I would injure the puppy by traction as it would
not be delivered so easily. Breeders have been known to pull off the
limbs or tails.
 If
I could not get the puppy out, it would die soon when the bitch bit
the water bag. The mother was not yet prepared for the Caesarian
operation. There would be at least 10 minutes to prepare her
for sterile and clean surgery and to make her sleep under
anaesthesia.
So, should I do it? The puppy head went in again. It might be a
large baby and might not come out of the birth canal so easily as I
thought. No, I would not take the risk of delivery one dead
puppy through forced traction.
I
put the anaesthetic gas mask into the bitch's nose. She slept and four large and
vigorous Shih Tzu puppies were taken out. Clean puppies without the
brownish stain of meconium
 (puppy
stools). None of them were weak or
stressed out and they should grow up big and strong.
This was one of the most satisfying
operations for a veterinary
surgeon. Puppies able to cry and breathe within 2 minutes of being
taken out of the water bag and to live past the critical first 6
weeks.
 The
mother was awake from anaesthesia in 2 minutes. A bit groggy as she
looked at the four big ones.
The pet shop owner had brought them home
specially to care for them. He had a bad experience of delay in one
earlier case and had learnt to get "prompt" veterinary surgery.
Still, it took him some thirty minutes to reach the surgery!
It is best to take a taxi as it is a matter of life and death in dystocia (difficulty in birth) cases!
